AGREEMENT FOR SERVICES

These services are detailed by way of specification, but not limitation, and by signing our service agreement you are agreeing to pay Eagle Resolutions & Resources International, Inc. (ERRII) and subsidiaries (EA Investigative Services (EIS), Security Operations, Executive (SOE) and the Center for Operational Studies and Training (COST)) invoices as rendered for fees and services. All services will be billed by Eagle Resolutions & Resources International, Inc. (ERRII) and subsidiaries regardless of which subsidiary rendered the service.

ERRII strives to serve you in the most cost effective and timely manner. As a result, ERRII personnel are very responsive to your needs. ERRII also offers several payment methods for the convenience, confidentiality, and security of the client.

INVESTIGATION/POLYGRAPH EXAMINATION RETAINER - A minimum retainer fee of $2,500.00 is required for long term investigations or for multiple polygraph examinations.The polygraph examination fees are subject to change based on the number of examinations conducted.

POLYGRAPH EXAMINATION NO SHOW/NO TEST FEES/LATE CANCELATION FEES - The regular polygraph examination fee will be charged if the examinee meets the EIS polygraph examiner and subsequently does not take the examination or if the polygraph examiner appears at an off-site location (such as a correctional facility) and is not allowed entry. There is no charge for polygraph examinations canceled or postponed prior to 9:00 A.M. on the proceeding business day. If the examinee fails to appear for the scheduled appointment and EIS has not been notified by a cancellation prior to 9:00 A.M. on the proceeding business day, a fee of one-half (1/2) the regular polygraph examination fee will be charged. APPOINTMENTS WILL NOT BE HELD OR CONFIRMED UNTIL EIS RECEIVES AN EXECUTED SERVICE AGREEMENT. YOU WILL ALSO AGREE TO PAY FOR NO SHOW/NO TEST FEES AS DETAILED ABOVE.

TYPING FEES - Reports are usually mailed within 5 working days at no additional charge. Expedited reports, required within 48 hours of the completion of an investigation or an examination, will be charged an additional $45 typing charge. Expedited reports which require typing on weekends or holidays will be charged an additional $75.

WITNESS FEES - EIS witness fees for appearance within Dade County is $750 for up to and including 3 hours, portal-to-portal, and $150 thereafter. EIS' witness fees for appearance outside of Dade County is $2,500.00, per 8 hour day, plus expenses. EIS wWitness fees will be billed to you if EIS' representative is subpoenaed by another party for related deposition, discovery, or other required services if that other party cannot be caused to pay our fee.

DEPOSIT REQUIREMENTS - A 50% deposit is required for all services provided by ERRII and subsidiaries at least 24 hours prior to the scheduled service. Courses taught by COST will require that the course balance be paid during the course. Deposits are non-refundable and nontransferable, valid catastrophic situations maybe taken into consideration for transfer of fees.

ADDITIONAL CHARGES - Fees are due upon receipt of invoice and interest at the rate of 1 1/2 % per month will be charged for any invoices not paid within 30 days. The client will be liable for the cost of all collection procedures, including attorney's fees and court costs. Venue for collections related legal action is agreed to be Broward County, Florida.

CREDIT CARDS

Keep your payment confidential and avoid the mention of a private investigation or polygraph on your monthly statement by paying using MasterCard or Visa. We have joined with Bank of America in order to provide our clients a secure online method of payment. The best part is your statement will show a payment for Bank of America rather than ERRII.

CHECK/MONEY ORDERS

We do not accept personal checks. We do accept company checks, out of state checks, and money orders. If confidentiality prevents the client from making the check payable to ERRII or contact our office for different options. No personal checks will be accepted as payment for Domestic Polygraphs. Checks returned for non sufficient funds (NSF) or other reasons will be referred to a collection agency and will be subject to the maximum returned check fee allowed by law. The initiation of an investigation may be delayed until the check clears.

CASH

Clients paying in cash will receive a receipt when making the payment in our offices.

WESTERN UNION

Cash can be sent by visiting one of the many Western Union Agent locations. A "To send money" form will need to be filled out with the following information: name, address, telephone number, receiver's name, location to be picked up, and the principal dollar amount. Please contact Eagle Resolutions & Resources International, Inc. and subsidiaries prior to initiating the transfer to obtain the name of the Case Manager who will be receiving the transfer. There will be a fee charged by Western Union that must be paid in cast at the time of the transaction.

After the transfer has been completed, contact ERRII to notify them of the transfer. The client must also provide the name and telephone number of the sender as listed on the transfer, as well, as the dollar amount.
